Use of MRI to predict symptomatic haemorrhagic transformation after thrombolysis for cerebral ischaemia

Caparros F, Kuchcinski G, Drelon A, et al. - Given that predictors of symptomatic haemorrhagic transformation (s-HT) of cerebral ischaemia following intravenous recombinant tissue-plasminogen activator (rt-PA) have been identified in studies utilizing CT scans, researchers ascertained if MRI could identify other predictors. In a cohort of consecutive patients who received intravenous rt-PA for cerebral ischaemia after MRI at baseline, predictors of s-HT were analyzed. Data reported that 49 of 944 patients had developed s-HT. Age, excessive alcohol consumption, recent transient ischaemic attack, and baseline national institutes of health stroke scale score were clinical factors independently associated with s-HT. The predictors for MRI were vascular hyperintensities, old infarcts, and abnormality in the volume of diffusion-weighted imaging (DWI). Although s-HT is predicted by the volume of DWI abnormality, other imaging features that can only be assessed with MRI were not associated significantly with s-HT.
