Unrecognized fluid overload during induction therapy increases morbidity in patients with acute promyelocytic leukemia

Chamoun K, et al. - Researchers examined cases of acute promyelocytic leukemia (APL) to determine the risk factors related to transfer to the intensive care unit (ICU) and endotracheal intubation during induction therapy. Overall 187 patients with newly diagnosed APL were included in this study. The participants had received treatment with all-trans retinoic acid and arsenic trioxide with or without gemtuzumab ozogamicin. In this APL patient populace, ICU transfer and endotracheal intubation occurred in relation to large volumes of blood product transfusions and unrecognized fluid overload during induction. With the implementation of certain steps such as limiting the amount of transfusions, careful monitoring for subtle signs of fluid overload, and early intervention with aggressive diuretic therapy, ICU transfer and endotracheal intubation can be avoided.
