Unplanned ICU admission is associated with worse clinical outcomes in geriatric trauma patients

Mulvey HE, et al. - Researchers sought to report on the overall rate and impact of unplanned ICU admission (UIA) in their geriatric trauma population. In addition, they sought for the predictive risk factors. From January 2012 to June 2018, they identified admission of 2,923 geriatric patients (≥ 65) at their urban, level I trauma center. Of these, UIA was reported in 95 (3.3%) patients, most commonly secondary to respiratory (34.7%) and cardiac (22.1%) events. On logistic regression, they identified age and injury severity score as predictive of unplanned ICU admission. Geriatric trauma patients requiring UIA exhibited relatively low injury severity but with significantly high morbidity and mortality. High risk for UIA was reported particularly for those who were initially admitted to the ICU, inferring the benefit of strategies to provide an extra layer of care post-ICU.
