Unexpected high rates of persistent Trichomonas vaginalis infection in a retrospective cohort of treated pregnant women

Lazenby GB, et al. - In this retrospective cohort study of women diagnosed with genital Trichomonas vaginalis from 2008 to 2017, the rate of persistent Trichomonas infection posttreatment was determined in pregnant women. In addition, researchers investigated if oral multidose metronidazole was associated with fewer cases of persistent Trichomonas compared with single-dose treatment. Five hundred forty-two women with 565 pregnancies were diagnosed with Trichomonas infection and were prescribed either single-dose (n = 352) or multidose metronidazole (n = 74). In 326 subjects, they collected posttreatment Trichomonas tests, 21 days or longer, that were positive in 44% (143). Women receiving single-dose and those receiving multidose regimens were similar regarding rates of positive Trichomonas tests. In view of high rates of positive Trichomonas tests and increased detection with nucleic acid amplification tests (NAATs), they recommend retesting all pregnant women with NAATs approximately 3 weeks posttreatment.
