Under-dilated tips associate with efficacy and reduced encephalopathy in a prospective, non-randomized study of patients with cirrhosis

Schepis F, et al. - Researchers analyzed patients with cirrhosis to examine whether polytetrafluoroethylene-covered stent grafts (PTFE-SGs) dilated to a smaller caliber [under-dilated trans-jugular intrahepatic porto-systemic shunt (TIPS)] reduced portosystemic encephalopathy (PSE) yet maintained clinical and hemodynamic efficacy. Moreover, they investigated whether under-dilated TIPS self-expanded to nominal diameter over time. It was noted that none of the PTFE-SGs self-expanded to the nominal diameter in hemodynamically relevant sites (such as portal and hepatic vein vascular walls). Under-dilation of PTFE-SGs during TIPS placement was noted to be feasible, associated with lower rates of PSE, and effective.
