Ultrasound detection of calcinosis and association with ulnar artery occlusion in patients with systemic sclerosis

Fairchild R, Chung M, Sharpless L, et al. - This research was attempted to assess the ability of ultrasound (US) compared to radiographs in detecting calcinosis in hands/wrists of systemic sclerosis (SSc) patients, and assess US markers of pathologic perfusion. Researchers analyzed SSc patients for calcinosis in the hands/wrists by x‐ray and US. They noted presence or absence of calcinosis by patient, hand, and anatomical zone; sensitivity and specificity for calcinosis detection by US versus x‐ray was ascertained. They further obtained bilateral US vascular measurements of ulnar artery occlusion (UAO) and finger pulp blood flow (FPBF). Generalized estimating equations were applied to evaluate associations between markers of pathologic blood flow (UAO, FPBF, and a composite severity score of UAO and FPBF) and presence of calcinosis for each hand. A total of 43 SSc patients were included in the study. US was was found to be sensitive and highly specific in detecting calcinosis in SSc. It was shown that UAO was strongly correlated with x‐ray identified calcinosis.
