Ultrasound-assisted vs landmark-guided paramedian spinal anaesthesia in the elderly: A randomised controlled trial
European Journal of Anaesthesiology Sep 11, 2019
Park SK, et al. - In this randomised controlled study, researchers compared ultrasound-assisted vs landmark-guided paramedian spinal anaesthesia in the elderly, focusing on the number of needle passes required for successful dural puncture. This inquiry was run from October 2017 to January 2018 at a single-institution, tertiary-level hospital in Seoul, Republic of Korea. Participants were 80 patients with at least 60 years of age undergoing orthopaedic surgery. Using either the landmark-guided or preprocedural ultrasound-assisted technique, paramedian spinal anaesthesia was administered in all participants. Findings revealed that the number of needle manipulations and periprocedural pain and discomfort scores were all reduced as a result of using the ultrasound-assisted paramedian technique vs landmark-guided paramedian technique in the elderly. Experts concluded that the performance of spinal anaesthesia in the elderly is facilitated by neuraxial ultrasonography.
