Two-stage revision total hip arthroplasty without spacer placement: A viable option to manage infection in patients with severe bone loss or abductor deficiency

Hipfl C, Carganico T, Leopold V, et al. - This study was sought to present the clinical course of a contemporary two-stage exchange for periprosthetic hip infection without spacer insertion. Researchers examined 141 infected total hip arthroplasty (THA) with extensive bone loss or abductor damage who had undergone two-stage exchange without spacer placement. Interim revision, reinfection, and aseptic revision rates were the clinical outcomes. They measured restoration of leg-length and offset radiographically. They further estimated modified Harris hip scores. For the management of chronically infected THA with severe bone loss or abductor deficiency, a non-spacer two-stage exchange was found to be a viable option, exhibiting comparable rates of interim revision and recurrence of infection. According to the findings, cementless reimplantation indicates good mid-term survivorship with comparable functional results and leg-length restoration. Nevertheless, dislocation continues to be a major concern.
