Twenty years of experience with less radical fertility-sparing surgery in early-stage cervical cancer: Oncological outcomes

Hruda M, Robova H, Rob L, et al. - Researchers conducted the largest single-institution study reporting 20 years of experience with two-step less radical fertility-sparing surgery in cervical cancer patients with IA1, LVSI positive, IA2 and IB1 (<2 cm, infiltration less than half of stromal invasions. Of 91 enrolled women (median age 29.1 years, range 21–40), 51 (56.0%) were nulliparous. Sentinel lymph node mapping and selective parametrectomy were performed in all women. Outcomes revealed very low recurrence rate, zero mortality rate. There were no distant recurrences, despite conducting all procedures laparoscopically. No use of cerclage was observed. Based on findings, they recommend performing extended and accurate follow-up and suggest human papillomavirus – high risk (HPV-HR tests) as valuable in such follow-up assessment.
