Treatment outcomes and attrition in gram negative periprosthetic joint infection

Kalbian I, et al. - A retrospective analysis of 1,189 PJI cases between 2007 and 2017 was done using the institutional PJI database in order to contrast the treatment outcomes for gram negative (GN) PJI vs gram positive (GP) PJI and gram mixed (GM) PJI. Treatment failure described by international consensus criteria was contrasted between PJI resulted by GN organisms (n = 45), GP organisms (n = 663), and GM (n = 28) cases. However, no noted variation in the overall international consensus failure rates between GN and GP PJI cases was found, there was important attrition in the two-stage exchange GN cohort, and these individuals were notably less inclined to reach reimplantation. The conclusions authenticated the current notion that GN PJI was related to poorer overall outcomes vs GP PJI. Moreover, this data contributed to the prevailing body of literature, which could recently underestimate the overall failure rates of GN PJI treated through two-stage exchange and failed to recognize prereimplantation morbidity.
