Transradial secondary access to guide valve implantation and manage peripheral vascular complications during transcatheter aortic valve replacement

Jackson MWP, et al. - Researchers compared their experience of transradial (TR) vs transfemoral (TF) secondary access during transcatheter aortic valve implantation (TAVI). Also, they described techniques for performing iliofemoral arterial intervention from the TR approach. Findings demonstrated safety as well feasibility of TR secondary access during TAVI. Moreover, this approach may reduce the secondary access site vascular complication rate. TR access, with appropriate equipment, could entirely manage most peripheral vascular complications avoiding unplanned femoral arterial access. Considering TR secondary access as the default approach for non-TF TAVI cases was recommended. In addition, this approach can be considered for all TF cases as long as dedicated equipment is available.
