Transcatheter aortic valve replacement and percutaneous coronary intervention vs surgical aortic valve replacement and coronary artery bypass grafting in patients with severe aortic stenosis and concomitant coronary artery disease: A systematic review

Catheterization and Cardiovascular Interventions Jul 17, 2020

Kotronias RA, Bray JH, Scarsini R, et al. - Via conducting a systematic review and meta‐analysis, researchers investigated the early and midterm outcomes of patients who underwent surgical aortic valve replacement (SAVR) and coronary artery bypass grafting vs those who had transcatheter aortic valve replacement and percutaneous coronary intervention. A comprehensive search of Medline and Embase yielded six studies reporting on 1,770 participants for inclusion in the meta‐analysis. Per findings, similar outcomes are attained using a percutaneous transcatheter approach vs a surgical approach in cases with severe aortic stenosis and coronary artery disease. However, as the studies were of low quality, the findings should serve as hypothesis generating. Researchers here emphasize individualized decision making based on surgical risk assessment in the absence of adequately powered studies yielding high level evidence.
