Total hip replacement for displaced intracapsular neck of femur fracture. Are current guidelines appropriate for all patients? Five-year retrospective analysis of 315 cases

Studnicka K, et al. - Given that total hip replacement (THR) was received by 31.4% of patients with displaced intracapsular neck of femur (NOF) fracture, who were viewed eligible by National Institute for Health and Care Excellence (NICE) for this procedure, in 2018 as per the National Hip Fracture Database (NHFD). Researchers aimed at recognizing the compliance of conducting total hip replacement (THR) for those patients in this unit and determining the reasons for proceeding with the alternative type of surgery. They conducted a 5-year retrospective review of 348 eligible cases. Findings revealed a continuously increasing amount of THRs performed for fractured NOFs, however, there were just over half of eligible patients who received it last year. Comorbidities were reported in nearly half of patients meeting the guidelines criteria of THR for trauma that outweigh the risk-benefit ratio for THR. The information on the preoperative decision making is not incorporated in NHFD. Improvement in credibility of annual reporting was observed with analysing such data. 100% compliance with NICE guidelines should not be achieved in the best interest of patients.
