Total hip arthroplasty or hemiarthroplasty for hip fracture

Bhandari M, Einhorn TA, Guyatt G, et al. - A total of 1,495 individuals who were 50 years of age or older and had a displaced femoral neck fracture were randomized to undergo either total hip arthroplasty or hemiarthroplasty in order to determine the impact of total hip arthroplasty in comparison with hemiarthroplasty, on a displaced femoral neck fracture. The trial was done in 80 centers in 10 countries. Mortality was comparable across the two treatment groups. Serious adverse events happened in 300 patients in the total hip arthroplasty group and in 265 individuals in the hemiarthroplasty group. The incidence of secondary procedures, among independently ambulating patients with displaced femoral neck fractures, did not vary significantly between patients, and total hip arthroplasty over hemiarthroplasty gave a clinically insignificant betterment in function and quality of life over 24 months.
