Tonsil size and outcome of uvulopalatopharyngoplasty with tonsillectomy in obstructive sleep apnea

Tschopp S, et al. - Researchers examined how tonsil volume and grade influence outcomes of uvulopalatopharyngoplasty (UPPP) with tonsillectomy in patients with obstructive sleep apnea (OSA). In this retrospective cohort analysis, they analyzed data of 70 consecutive patients undergoing UPPP with tonsillectomy between 2015 and 2018. Outcomes revealed significant correlation of tonsil grade and volume with a preoperative apnea-hypopnea index (AHI). They identified higher preoperative AHI values with large tonsils, and their removal leads to a greater reduction of initial AHI. However, there appeared no significant correlation of tonsil size and volume with the postoperative effect on daytime sleepiness and snoring reduction, intimating that these parameters are mainly influenced by other factors. When counseling OSA patients, ear, nose, and throat physicians are suggested to be aware of the significance of tonsil size and volume.
