TIMP-2/IGFBP7 predicts acute kidney injury in out-of-hospital cardiac arrest survivors

Adler C, et al. - Whether early recognition of acute kidney injury (AKI) in patients with non-traumatic shock is possible by evaluation of novel urine biomarkers tissue inhibitor of metalloproteinases-2 (TIMP-2) and insulin-like growth factor-binding protein 7 (IGFBP7) was investigated. Patients with shock following out-of-hospital cardiac arrest (OHCA) were treated with target temperature management (TTM) for 24 hours. At 3 and 24 hours after determination of OHCA, urinary [TIMP-2]·[IGFBP7] samples were collected. With a cut-off at 0.24, urinary [TIMP-2]•[IGFBP7] allowed a reliable prediction of AKI in high-risk patients only 3 hours after determination of OHCA. Patients at high-risk of AKI may be identified using this novel test.
