Timing of transversus abdominis plane block and postoperative pain management

Escudero-Fung M, et al. - As postoperative pain and opioid consumption after surgery can be reduced with performing transversus abdominis plane (TAP) blocks using liposomal bupivacaine, researchers sought to ascertain how the timing of administration of such blocks affects. They conducted a retrospective cohort study of 287 adult patients that underwent colorectal procedures and received TAP blocks with liposomal bupivacaine. Of these patients, 71 were provided blocks prior to induction of general anesthesia (pre-ind), 85 were provided blocks after induction of general anesthesia but prior to surgical incision (post-ind) and 131 were provided blocks after completion of surgery (post-op). Per outcomes, there appeared no difference if TAP blocks with liposomal bupivacaine are carried out before or after induction of anesthesia. Consideration should be given to patient preference and operating room efficiency when taking decisions concerning the timing of these blocks.
