Timing and efficacy of mifepristone pretreatment for medical management of early pregnancy loss

Flynn A, Roe AH, Koelper N, et al. - Researchers conducted a secondary analysis of a randomized trial with the aim to identify the time interval between mifepristone and misoprostol administration that is linked with the most efficacious early pregnancy loss (EPL) management. Participants with early pregnancy loss (EPL) were asked to take 200 mg oral mifepristone followed by 800 mcg vaginal misoprostol 24 hours later. One hundred thirty nine participants were identified as eligible; of these, 70 (50.4%) self-administered misoprostol before 24 hours, and 69 (49.6%) at or after 24 hours. Time intervals were defined as follows: 0-6 hours (n = 22); 7-20 hours (n = 29) and 21-48 hours (n = 88). Outcomes suggested the highest likelihood of success in medical management of EPL when misoprostol is self-administered 7-20 hours after mifepristone.
