Time-to-death in chronic respiratory failure on home mechanical ventilation: A cohort study

Schwarz EI, Mackie M, Weston N, et al. - Researchers performed a systematic analysis of the time as well as the circumstances of death on home mechanical ventilation (HMV). They used prospectively obtained all-cause mortality data of HMV patients from a large tertiary centre. Diagnostic groups defined based on data were: neuromuscular disease (NMD), chest wall disease (CWD), chronic obstructive pulmonary disease (COPD), obesity hypoventilation syndrome (OHS), overlap syndrome of COPD and OSA (overlap) and other group. Time-to-death from the start of HMV was the primary outcome. The reported median time-to-death was 19.5 [6–55] months. The longest time-to-death on HMV was reported for CWD and slowly progressive NMD, while a longer median time-to-death was observed in OHS and overlap syndrome vs COPD and motor neurone disease. Better outcomes were achieved with daily adherence to HMV of greater than 4 h/night. Overall, findings revealed a wide variation in the time-to-death on HMV across disease groups with chronic respiratory failure, and the time-to-death on HMV appeared to be related to daily usage time.
