The Yonsei experience of 104 laparoscopic pancreaticoduodenectomies: A propensity score-matched analysis with open pancreaticoduodenectomy

Han SH, et al. - The safety and feasibility of laparoscopic pylorus-preserving pancreaticoduodenectomy (L-PPPD) vs open PPPD (O-PPPD) were compared. From September 2012 to June 2017, a single surgeon performed PPPD on 217 patients (O-PPPD on 113 and L-PPPD on 104) at Yonsei University Severance Hospital. Compared to the O-PPPD group, the L-PPPD group had lesser intraoperative blood loss, similar rates of negative resection margins, no significant differences regarding overall complication rates and no difference in 30- and 90-day mortality rates. In the O-PPPD and L-PPPD groups, the clinically relevant postoperative pancreatic fistula rates were 18.8% and 13.5%, respectively. These findings support that for well-selected patients with periampullary lesions requiring PPPD, L-PPPD can be a good alternative option.
