The variable impact of aneurysm size on outcomes after open abdominal aortic aneurysm repairs

Mehta A, O’Donnell TFX, Trestman E, et al. - The correlation between aneurysm size with outcomes after open repairs was investigated via appraising data of patients who underwent open repairs of infrarenal vs juxtarenal non-ruptured abdominal aortic aneurysms (AAAs), defined by proximal clamp site, in the 2004-2019 Vascular Quality Initiative. A total of 8,011 patients were identified (54% infrarenal, 46% juxtarenal). Infrarenal and juxtarenal aneurysms did not differ in median aneurysm size (5.7cm vs 5.9cm). Every one-centimeter increase in size of infrarenal aneurysm was correlated with raised adjusted odds or hazards ratio of 30-day mortality by 18%, failure-to-rescue by 20%, 1-year mortality by 18%, but not complications. For juxtarenal aneurysm, no correlation of larger aneurysm sizes with any outcome was observed. Association of proximal clamp site, ischemia time, and volume with outcomes was observed. These findings suggest less relevance of the correlation between AAA size and outcomes with involvement of renal and visceral artery aneurysm, having important implications for surgical decision-making, operative planning, and patient counseling.
