The Shock Index is not accurate at predicting outcomes in patients with upper gastrointestinal bleeding

Saffouri E, Blackwell C, Laursen SB, et al. - In an international, prospective study involving 3,012 consecutive patients, researchers compared the Shock Index with existing preendoscopic upper gastrointestinal bleeding (UGIB) risk scores in anticipating outcomes following UGIB. Among UGIB patients, they compared the Shock Index with existing scores including the Glasgow Blatchford score, admission Rockall score, AIMS65, and the newly described “ABC” score. In predicting major clinical endpoints after UGIB, the Shock Index performed poorly with AUROCs < 0.66 and was lower than existing preendoscopy scores. Despite the National Confidential Enquiry into Patient Outcome and Death suggestion, the authors did not find any clear evidence that the Shock Index is clinically useful in predicting UGIB results.
