Riordan F, et al. – This trial ascertained if the hospital and community diabetes nurse specialist (DNS) assisted in the integration of diabetes care, analyzed the regional change in terms of service relevant to the delivery of integrated care and determined the barriers to its delivery and areas of improvement. The data revealed that in order to deliver integrated diabetes care, the following ought to be explicitly targeted: Wider service issues, regional disparities in access to specialist resources and formalizing agreements and protocols on multidisciplinary working between settings.
