The role of mpMRI in qualification of patients with ISUP 1 prostate cancer on biopsy to radical prostatectomy

Nyk L, Tayara O, Ząbkowski T, et al. - This study was carried out to assess the role of mpMRI and high prostate imaging and report and data system (PIRADS) score as independent triggers in the qualification of patients with ISUP 1 prostate cancer on biopsy to radical prostatectomy. Researchers conducted 494 laparoscopic radical prostatectomies in the institution, including 203 patients (41.1%) with ISUP 1 cT1c-2c prostate cancer on biopsy between January 2017 and June 2019. They retrospectively examined biopsy results, digital rectal examination, PSA, mpMRI, and postoperative pathological report. The results showed that in patients unwilling to repeat biopsies, PIRADS score is associated with an upgrade on final pathology and may justify the shared decision of radical treatment. According to the findings, the use of the PIRADS 5 score as a sole indicator for prostatectomy may result in a nonnegligible overtreatment rate.
