The role of comorbidities in benign paroxysmal positional vertigo

Sreenivas V, et al. - In this descriptive analytical study, researchers evaluated the relationship between the comorbidities, like hypertension, diabetes, thyroid disorders, hearing loss, hyperlipidemia, and vitamin D deficiency and benign paroxysmal positional vertigo (BPPV) and determined the high-risk groups for recurrence of symptoms. Including Dix-Hallpike test and roll-over test and blood pressure recording, patients who met the inclusion criteria had complete ear, nose, and throat examination. The older age group has a higher risk of BPPV and symptom recurrence. Increased risk of BPPV and its recurrence was discovered for diabetes mellitus. Other comorbidities, like abnormal thyroid function test, sensorineural hearing loss, hypercholesterolemia, and vitamin D deficiency, showed no significant risk of recurrence. The presence of comorbidities worsens BPPV status, resulting in more frequent detachment of otolith. Thus, even after a successful repositioning maneuver, it increases the risk of recurrence. Therefore, patients with BPPV should be assessed and treated for these comorbidities along with the repositioning maneuvers.
