The relationship between ICU hypotension and in-hospital mortality and morbidity in septic patients

Maheshwari K, et al. - In this retrospective analysis, researchers assessed the associations of mean arterial pressures (MAPs) below various thresholds and in-hospital mortality, acute kidney injury (AKI), and myocardial injury in septic adults in US hospitals, with intensive care unit (ICU) stays ≥ 24 h from 2010 to 2016, excluding patients with inadequate blood pressure recordings, poorly documented potential confounding factors, or renal or myocardial histories documented within 6 months of ICU admission. They found that at 85 mmHg, risks for mortality, AKI, and myocardial injury were apparent. Progressive worsening of mortality and AKI risk was noted at lower thresholds. In this patient population, it may be prudent to maintain MAP well above 65 mmHg.
