The prognostic implication of perioperative cardiac enzyme elevation in patients with fractured neck of femur: A systematic review and meta-analysis

Lowe MJ, et al. - In view of the observation that comorbid elderly patients frequently suffer neck of femur (NOF) fracture which is associated with increased rates of morbidity and mortality following the fracture and that the VISION cohort yielded evidence that troponin rise in the postoperative period after a diverse range of non-cardiac surgical procedures may result in an enhanced risk of mortality in the nonexistence of classical ischaemic or cardiac symptoms, researchers here sought to ascertain whether perioperative troponin rise is valuable as a prognostic indicator for mortality and cardiac morbidity in those with fractured NOF via performing this systematic review and meta-analysis. Performing an electronic search of the EMBASE, MEDLINE (Ovid) and Biosis databases, they identified 11 studies that met inclusion criteria giving a total of 1,363 patients; of these, 497 patients (36.5%) were reported to have an elevation in troponin levels following surgery. Outcomes revealed a significant correlation of perioperative troponin elevation with increased mortality and postoperative cardiac complications following fractured NOF. Hence it may serve as a useful prognostic indicator in these patients.
