The prevalence of pacing-induced cardiomyopathy (PICM) in patients with long term right ventricular pacing- Is it a matter of definition?

Kaye G, et al. - In patients with chronic right ventricular pacing, researchers determined the incidence of pacing-induced cardiomyopathy (PICM). Excluding patients with cardioverter-defibrillators or biventricular devices, this study included patients attending a pacemaker clinic with transthoracic echocardiographic left ventricular ejection fraction (LVEF) measurement during the 12 months prior to device implantation. Use of three different definitions of PICM was reported, according to LVEF. They found that, based on the PICM definition, the prevalence of PICM ranged from 5.9 to 39.0%. A significant association of PICM with ventricular pacing burden was revealed in multivariate analysis. Overall, the prevalence of PICM was dependent on current accepted clinical definitions.
