The need for an antibiotic stewardship program in a hospital using a computerized pre-authorization system

Şengel BE, et al. - Researchers evaluated the effect of a prospective audit and feedback strategy system along with the nationwide antibiotic restriction program (NARP) via performing this prospective quasi-experimental study. An antimicrobial stewardship program (ASP) strategy included a computerized pre-authorization system to approve the antibiotics. They monitored patients with intravenous (IV) antibiotic use ≥72 h without intervention during the baseline period. In the second period, attending physicians received feedback and treatment recommendations in the case of IV antibiotic use ≥72 h. For appropriateness of prescribing, modified criteria of Kunin et al. and Gyssens et al. were followed. They observed a total of 866 antibiotic episodes among 519 patients and identified a significant reduction in systemic antibiotic consumption in the intervention period. The antimicrobial restriction program alone was efficacious. However, a tailored ASP, such as prospective audit and feedback, should support the system.
