The impact of spinal needle selection on postdural puncture headache: A meta-analysis and metaregression of randomized studies

Zorrilla-Vaca A, et al. - In this review, researchers assessed the link between spinal needle characteristics and incidence of postdural puncture headache (PDPH). They analyzed data from randomized controlled trials that were focused on the impact of needle design and gauge on the incidence of PDPH. After controlling for patient confounders such as age, sex, and year of publication, meta-analysis and metaregression were performed. They found relative to the cutting-needle design, pencil-point needles were related to significantly lower incidence of PDPH. For cutting-needle design, needle gauge was found to be significantly associated with PDPH but a similar link was not shown for pencil-point needles. Selection of larger-caliber pencil-point needle was recommended to maximize technical proficiency without expensing increased rates of PDPH.
