The impact of pretransplant biopsy on 5‐year outcomes of expanded criteria donor kidney transplantation

Yap YT, Ho QY, Kee T, et al. - Researchers conducted a single‐center retrospective study investigating 5‐year outcomes of deceased‐donor kidney transplant recipients, comparing recipients of expanded criteria donor (ECD) allografts evaluated histologically with recipients of SCD and ECD kidneys assessed clinically. From November 2005 to December 2009 (Era 1), clinical assessment of donors for suitability for kidney donation was undertaken. From December 2009 to October 2017 (Era 2), biopsy was performed on kidneys from ECDs and diabetics prior to transplantation and their allocation was done based on Remuzzi score. From Era 1 to 2, there was an increase in ECD kidney transplantation from 30.4% to 40.0%. Outcomes suggest association of introduction of pre‐transplant biopsy assessment with improved outcomes of ECD kidney recipients such that they are now comparable to SCD kidney recipients, with benefits persisting over 5 years.
