The impact of hemodialysis on mortality and personal independence after hip fracture. A prospective matched cohort study

Orabona N, Bove A, Smeraglia F, et al. - In this prospective, observational cohort study (a consecutive cohort of 64 individuals with end-stage renal disease who were receiving chronic hemodialysis who had undergone surgery for a trochanteric or femoral neck fracture from June 2008 to November 2016) and with matched controls (individuals without end-stage renal disease who underwent surgery for comparable hip fractures), researchers ascertained the cumulative 30-day and 1-year mortality as well as personal independence following hip fracture in individuals on hemodialysis. In individuals with a hip fracture undergoing hemodialysis, the 30-day and 1-year mortality rate was 25.0% and 57.8%, respectively. In contrast with the matched controls, hemodialysis was independently related to raised 30-day and 1-year mortality. At the 1-year follow-up, compared with the prefracture status, loss of personal independence was discovered. Thus, hemodialysis was related to elevated mortality following hip fracture. Furthermore, a worse prefracture functional status prognosticated the loss of functional independence at follow-up.
