The factors contributing to efficiency in surgical management of purulent infections of deep neck spaces

Rzepakowska A, Rytel A, Krawczyk P, et al. - Via performing a retrospective analysis of medical data of 46 patients (age ranged from 18 to 82 years), who had surgical treatment in the department due to purulent deep neck infection (DNI: defined as bacterial infection in the potential spaces and fascial planes of the neck, including submandibular, lateral cervical, visceral, carotid, parapharyngeal, and retropharyngeal spaces) in the period from 2009 to 2017, researchers evaluated the effectiveness of the treatment and identified the prognostic factors that contribute to complications and prolonged hospitalization. Submandibular space, followed by parapharyngeal space, was the most common site involved. Most patients had single abscess, the multiple abscesses in 8 (18.8%) cases were reported. Data reported that the mean size of the abscesses was 42 mm. The most common species cultured from swabs were Staphylococcus aureus and Peptostreptococcus. The most widely used antibiotic treatment was a mixture of ceftriaxone and metronidazole. The relationship between different variables and hospitalization duration showed only the level of hemoglobin below 12.5 g/dL, a significant predictor of longer hospitalization. Patients with purulent DNIs and reduced levels of hemoglobin are predisposed following surgical drainage to delayed recovery.
