The effect of rocuronium on ventilatory leak and sealing pressure using a supraglottic airway device: A randomized clinical trial

Fujimoto M, et al. - Given that there is a lack of clarity regarding the transition of ventilatory leak and sealing pressure in supraglottic airway devices following delivery of neuromuscular blocking agents, researchers investigated if ventilatory leak would reduce because of the rise in sealing pressure following administration of 0.15 mg/ kg and 0.30 mg/kg rocuronium iv. This study involved overall 40 patients. The participants were randomized to a control group or one of two rocuronium groups. Following general anaesthesia induction, an i‐gel was inserted prior to rocuronium delivery. Immediately, measurements of ventilatory leak and sealing pressure were obtained. Then, ventilatory leak and sealing pressure were measured again after administration of 0.15 mg/kg or 0.30 mg/kg rocuronium iv. In the control group, just following insertion of the supraglottic airway device and 5 minutes later, measurements were taken, without any additional drug delivery. Findings revealed reduction of ventilatory leak with a supraglottic airway device, independently of neuromuscular blockade, although decrease in sealing pressure may potentially be induced by neuromuscular blockade.
