The effect of preoperative chemotherapy and chemoradiotherapy on pancreatic fistula and other surgical complications after pancreatic resection: A systematic review and meta-analysis of comparative studies

van Dongen JC, Wismans LV, Suurmeijer JA, et al. - Researchers conducted this systematic review and meta-analysis to determine the safety of preoperative chemotherapy and chemoradiotherapy in terms of major morbidity and mortality after pancreatic resection for pancreatic cancer, as well as the impact of preoperative therapy on pancreas-specific complications, specifically grade B/C postoperative pancreatic fistulas (POPF). This investigation involved studies reporting on the rate of surgical complications after preoperative chemo- or chemoradiotherapy vs immediate surgery in pancreatic cancer patients. A total of 41 comparative studies with a total of 25,389 patients were included. When compared with immediate surgery, preoperative chemo- and chemoradiotherapy seems to be safer in terms of POPF and other surgical complications in patients with pancreatic cancer. Preoperative chemoradiation appears to be responsible for the lower POPF rate.
