The effect of preinjury anticoagulation on mortality in trauma patients: A systematic review and meta-analysis

Lee ZX, Lim XT, Ang E, et al. - In this study, the impact of preinjury anticoagulation on mortality in trauma patients were evaluated. Researchers carried out to search of electronic information sources to distinguish all observational studies comparing preinjury anticoagulation with no preinjury anticoagulation in trauma patients. The primary endpoint included overall mortality (overall mortality, in-hospital mortality and 30-day mortality). The length of hospital stay, length of intensive care unit stay, incidence of intracranial haemorrhage, and need for operation were considered as secondary endpoints. They applied fixed-effect or random effects modelling as appropriate to estimate pooled outcome data. They enrolled 19 comparative studies enrolling a total of 1,365,446 patients. In trauma patients, preinjury anticoagulation is a significant prognosticator of mortality. Future studies should concentrate on strategies needed to decrease such a significant risk of mortality in these high-risk patients. For patients on preinjury anticoagulation, this may include adaptation of primary, secondary and tertiary trauma surveys.
