The effect of glenosphere lateralization and inferiorization on deltoid force in reverse total shoulder arthroplasty

Nolte PC, Miles JW, Tanghe KK et al, et al. - This study was intended to assess the impact of glenosphere lateralization and inferiorization on the deltoid force in reverse total shoulder arthroplasty. Researchers dissected eight cadaveric shoulders leaving only the rotator cuff muscles and capsule intact. They tested five consecutive testing states: intact shoulder, posterosuperior rotator cuff tears (PS-RCT), reverse total shoulder arthroplasty (RTSA) with standard glenosphere, RTSA with 4mm lateralized glenosphere, RTSA with 2.5mm inferiorized glenosphere. They applied Statistical Parametric Mapping (SPM) to examine the deltoid force as a function of the abduction angle. In comparison with standard or 4mm lateralized glenospheres, RTSA with a 2.5mm inferiorized glenosphere and no additional lateralization resulted in less deltoid force to abduct the arm. Thus, when intending to mitigate the downsides of a medialized center of rotation, an inferiorized glenosphere may be preferable in terms of its impact on the deltoid force.
