The effect of frailty on the 24‐hour blood pressure pattern in the very elderly

Blauth FG, dos Santos Vilar LA, de Carvalho Brito Pontes V, et al. - Findings demonstrate a substantial impact of frailty on nighttime blood pressure (BP) values and patterns in patients older than 80 years.
Frailty has an important role in hypertension management in the very elderly and has a strong link with cardiovascular diseases.
This study included patients older than 80 years; 38 were frail and 36 non-frail (mean age 85.3 ± 3.7 years; 67% females).
For frail and robust participants, similar were awake systolic and diastolic BP.
Frail patients exhibited higher systolic BP during sleep (128 ± 15 mm Hg vs 122 ±13 mm Hg) and decreased systolic BP decline [1 (-4.5 – 5)% vs 6.8 (2.1 – 12.8)%].
An independent association of frailty with higher risk of non-dipping (OR 12.4) and decreased nighttime systolic BP fall (-6.1%) was found.
