The effect of dexamethasone on postoperative blood glucose in patients with type 2 diabetes mellitus undergoing total joint arthroplasty

Allen D, et al. - Researchers examined whether administration of preoperative dexamethasone to diabetics is significantly associated with hyperglycemia and increased insulin requirements in the immediate postoperative period following total joint arthroplasty (TJA). In addition, they sought for risk factors for postoperative hyperglycemia immediately following TJA. In this retrospective review of 285 patients with type 2 diabetes who underwent TJA from 2010 to 2015, no correlation of dexamethasone administration with the maximum postoperative blood glucose was observed. They observed an initial increase in blood glucose and increased insulin requirement on post op day 0 in correlation to preoperative dexamethasone administration to these patients. However, the recognized effect on glucose control in patients with diabetes may not outweigh the known clinical advantages of peri-operative glucocorticoids.
