The DIFFMASK score for predicting difficult facemask ventilation: A cohort study of 46,804 patients

Lundstrøm LH, et al. - Using a cohort of 46,804 adult patients who underwent facemask ventilation during general anaesthesia, researchers developed and assessed a weighted risk score for predicting difficult facemask ventilation during anaesthesia. Using two consecutive steps, they developed the Difficult Facemask (DIFFMASK) score. With a sensitivity and specificity of 85% and 59%, respectively, difficult facemask ventilation can be predicted at an optimal cut-off value ie, a sum score ≥ 5, as indicated by the Youden index. Heightened attention during the planning of facemask ventilation may be needed in cases with a score of 6–10 points, as indicated by the DIFFMASK score, compared with patients who are obviously at a high- or low risk of difficulties. The possible utility of the DIFFMASK score was suggested in a clinical context.
