The association between ward staffing levels, mortality and hospital readmission in older hospitalized adults, according to presence of cognitive impairment: A retrospective cohort study

Fogg C, Bridges J, Meredith P, et al. - Via performing a retrospective cohort study, researchers sought to determine how registered nurse (RN) and nursing assistant (NA) staffing levels influence mortality and readmission in older patients admitted to general medical/surgical wards. Inclusion of 12,544 admissions was done. Similar levels of exposure to staffing were there for patients with  cognitive impairments (CI) (33.2%) to those without. 10% reduction in the odds of death, overall, was observed with an additional 0.5 RN hours per day: 15% in patients with CI and 7% in patients without. Findings suggest that despite similar exposure to low staffing, there was greater impact on mortality and readmission for patients with CI. They noted elevated mortality with higher NA staffing among patients without CI, which needs exploration.
