The acceptability and feasibility of using mortality prediction scores for initiating end-of-life goals of care communication in the adult intensive care unit

Orr S - Researcher investigated if providers’ use of patient mortality prediction scores as part of routine practice to increase prognosis precision and timeliness of end-of-life (EOL) communication is acceptable and feasible. In addition, the researcher evaluated providers’ intentions to change practice related to EOL goals-of-care communication based on awareness of the scores. Using an explanatory mixed-methods approach, Sequential Organ Failure Assessment (SOFA) patient mortality prediction scores were provided to ICU providers, who then completed an acceptability and feasibility questionnaire and engaged in follow-up interviews carried out to further understand questionnaire responses and obtain insight into their perceptions based on having SOFA scores. As per the analysis, providers with limited ICU experience identify the use of SOFA scores as a means to potentially increase EOL goals-of-care communication as most beneficial and acceptable. While those with more experience identified the scores as a supplement to their own intuition, although all acknowledged the benefit of looking at score ‘trends.’
