Ten-year impact of pancreaticoduodenectomy on bowel function and quality of life of patients with ileal pouch-anal anastomosis for familial adenomatous polyposis

Collard MK, Lefevre JH, Ahmed O, et al. - In this study, researchers assessed the long-term consequences of the combination of pancreaticoduodenectomy (PD) + ileal pouch-anal anastomosis (IPAA) as patients with family adenomatous polyposis (FAP) are at risk for duodenal adenocarcinoma. Participants in the study were all patients with IPAA due to FAP who had PD from 1991 to 2017 (PD + IPAA group). Study participants were matched 1:1 according to age, gender, ASA score and presence of colorectal cancer. Using the Wexner score and the EORTC-QLQ-C30 questionnaire, fecal continence and quality of life (QoL) were evaluated. For this investigation, they matched 32 PD + IPAA patients with 32 IPAA-only patients. Data reported that the response rate to the questionnaire was 78% in each group. In terms of daytime fecal frequency, Wexner score and global QoL score, there were no differences between PD + IPAA and IPAA-only groups. It was noted that 26% of individuals developed diabetes a median period of 10.0 years after PD in the PD + IPAA group. For these patients, the global QoL was significantly altered, while the daytime fecal frequency was unchanged as fecal continence. Findings suggested that PD has no effect on bowel function or QoL in FAP patients with IPAA.
