Systematic review: Efficacy of escalated maintenance anti-tumour necrosis factor therapy in Crohn disease

Mattoo VY, Basnayake C, Connell WR, et al. - Researchers sought to perform a systematic review evaluating the effectiveness of escalated maintenance anti-TNF therapy in Crohn disease (CD). The databases EMBASE, MEDLINE, Web of Science, and CENTRAL were searched for English language publications through April 25, 2021. A total of 4,733 records were found, with 68 articles meeting the eligibility criteria. Clinical response rates (33%-100%) and remission rates (15%-83%) following empiric dose escalation for loss of response to standard anti-TNF therapy were high but varied across studies. In the majority of patients with secondary loss of response to standard maintenance doses, empiric escalation of anti-TNF therapy can recapture clinical response. Proactive maintenance dosing optimization may extend the time to loss of response in some patients.
