Systematic review and updated network meta-analysis of randomized controlled trials comparing open, laparoscopic-assisted, and robotic distal gastrectomy for early and locally advanced gastric cancer

Aiolfi A, Lombardo F, Matsushima K, et al. - By performing systematic review and network meta-analyses of randomized controlled trials, researchers did a comprehensive assessment of major surgical approaches for operable distal gastric cancer. They compared open distal gastrectomy, laparoscopic-assisted distal gastrectomy, and robotic distal gastrectomy. They analyzed 17 randomized controlled trials (5,909 patients). In terms of 30-day mortality, anastomotic leak, and overall complications, no significant differences were found among these 3 approaches. Even in the setting of locally advanced gastric cancer, improved short-term outcomes as well as similar overall and disease-free survival seemed to be conferred by laparoscopic-assisted distal gastrectomy and robotic distal gastrectomy performed by well-trained experienced surgeons, when compared with open distal gastrectomy.
