Survival and prognostic factors of adult intracranial ependymoma: A single- institutional analysis of 236 patients

Zhao F, Wu T, Zhang J, et al. - Researchers sought to determine potential prognostic indicators as well as rational therapeutic management in a large cohort of adult intracranial ependymomas (EPNs). This study involved adult patients who had resection of World Health Organization grade II or III intracranial EPNs. There were 236 adult patients with a mean age of 36.2 years (range: 18 to 72 y) at diagnosis. In 169 patients (71.6%), gross total removal was achieved. Follow-up revealed disease progression in 97 (41.1%) patients and deaths of 39 (16.5%) patients. In Kaplan-Meier analysis, excellent survival was seen in patients with H3K27me3-positive posterior fossa ependymoma (PF-EPN), whereas poor prognosis was observed in patients with RELA fusion-positive supratentorial EPN or H3K27me3-negative PF-EPN. In multivariate analysis, the following were revealed as strong independent prognostic indicators: molecular subgroup, extent of resection, and Ki-67 index. Findings afford important data regarding the prognostic prediction of adult intracranial EPNs that will help in establishing suitable risk stratification as well as individualized treatment approaches in future clinical trials.
