Survival after an acute heart failure admission: Twelve-month outcomes from the NSW HF Snapshot Study

Newton PJ, et al. - Researchers describe the 30-day and 12-month outcomes from the New South Wales (NSW) Heart Failure Snapshot Study, which was conducted to offer a contemporaneous profile of patients admitted with acute heart failure. Consecutive patients with acute heart failure who were admitted to 24 teaching hospitals across NSW and the Australian Capital Territory from July–August 2013 were prospectively examined. Recruited patients were 811 in total. The NSW HF Snapshot included elderly individuals (77 ± 14 yrs) having high comorbidity and 71% were frail at baseline. Within 30-days after discharge, readmission was sought by 24% of patients. Deaths of 178 patients were reported within 12 months following discharge. Frailty Charlson Comorbidity Index; NYHA class 4; eGFR < 30 ml/min/1.73 m2; hypokalaemia at discharge and readmission within 30 days of baseline admission were all identified as independent predictors of death. Findings revealed high short-term readmissions and mortality at 12 months, which were extensively driven by patient-level factors, in this one of the largest prospective audits of acute heart failure outcomes in Australia.
