Surgical complexity score and role of laparoscopy in women with advanced ovarian cancer treated with neoadjuvant chemotherapy

Davidson BA, et al. - In this multi-institutional study including advanced epithelial ovarian cancer (EOC) patients receiving neoadjuvant chemotherapy (NACT), researchers assessed surgical complexity scores (SCS) and minimally invasive surgery (MIS) at interval debulking surgery (IDS). They obtained demographic data and assigned SCS, taking into account IDS findings. They also performed univariate and multivariate survival analyses by using Cox proportional hazards regression models. They identified a total of 282 patients, with high-grade serous histology in 80.5% and 54.6% were < 75. Worse survival in women with advanced EOC treated with NACT was predicted by older age, SCS ≥ 3, and residual disease >1 cm at IDS. Safety, as well as the feasibility of MIS, was shown in this study, with acceptable optimal cytoreduction rates.
