Short-term outcomes of transcatheter aortic valve implantation vs surgical aortic valve replacement in kidney transplant recipients (from the US nationwide representative study)

Abdelfattah OM, Saad AM, Abushouk A, et al. - Researchers here examined the outcomes of transcatheter aortic valve implantation (TAVI) vs surgical aortic valve replacement (SAVR) for aortic stenosis (AS) in patients with prior history of functioning kidney transplant. From the Nationwide Readmissions Database, they identified 762 TAVI and 1,278 SAVR kidney transplant recipients (KTRs) for inclusion in this study. Generally, rates of comorbidities were higher and risk of in-hospital mortality, blood transfusion, acute myocardial infarction, acute kidney injury, sepsis and discharge with disability, was lower in TAVI patients relative to those with SAVR. However, TAVI group had a significantly higher rate of permanent pacemaker implantation. Of note, both the groups were comparable in terms of in-hospital stroke and 30-day readmission. Based on finding, they suggest TAVI to be a valid and safe alternative for KTRs with severe AS.
