Wang M, Fan R, Gu T, et al. - Researchers used a multicenter database to assess the early prognosis as well as treatment of acute coronary involvement (ACI) in type A aortic dissection (ATAAD) patients without myocardial ischemia (MI) in this retrospective cohort analysis. They analyzed 931 ATAAD patients without MI who had thoracic aortic surgery between 2018 and 2019. Participants were split into two groups: ACI group and non-ACI group. A higher 30-day mortality post-surgery was observed in the ACI group vs the non-ACI group, particularly in sub-group of advanced age, low diastolic blood pressure, low systolic blood pressure, high body mass index and high hemoglobin. More significant acute kidney failure was present in the ACI group vs non-ACI group. Overall, an increase in the short-term postoperative mortality and acute renal failure was reported in relation to the presence of ACI in ATAAD patients without MI. A narrower control range of blood pressure may be required in ATAAD patients with ACI even if without myocardial ischemia.
